Advance care planning (ACP) is the process of thinking about, talking about and planning for future health care and end-of-life care.
What is advance care planning?
Advance care planning helps you, the important people in your life and your health care team plan for your end-of-life care.
It helps you understand what the future might hold, and to say what health care you would or would not want. This makes it much easier for everyone to know what you want - especially if you can no longer speak for yourself.
Start an advance care plan
There is no wrong or right way to capture this information. You only need to fill in the bits you want to. If you want to complete Section 6, this is best completed with help from a health care professional.
Once you have captured everything you want, share this with your general practice team and keep a copy with you. You’ll need to take this with you if you go into care or a hospital.
